Health Physical Appraisal Letter 2026
Dear Parent/Guardian:
As you were reminded with similar letters periodically from the end of last school year until now, New York State Law Section 2164 requires students entering Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grade to have a current “physical” on file with the school. Likewise at the secondary level, in order to participate on an athletic team the student must have a current physical on file.
Regardless of your reason for needing a physical, as of January 31, 2021 only the attached form is permitted to be used. While we at West Babylon have been using this form for years, the phase in period is over and no other forms are acceptable. Please check with your health care provider as soon as possible to make an appointment and work with us to meet this requirement. For the state approved form visit our website or open this link
