SHS Health Physical Immunizations

August 2026

Dear Parent/Guardian:

New York State Law Section 2164 requires new entrants and students entering 9th and 11th grade to have a current health appraisal a.k.a. "physical" on file with the school. Any physical performed must be on the Required NYS Health Examination or EHR equivalent form. A blank copy of this form is enclosed for your convenience.

Also understand that:

• The health examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner.
• The date of the complete physical exam must be after July 1, 2025.
• A dental certificate which states your child has been seen by a dentist or dental hygienist is also encouraged.

A copy of the health examination must be submitted to the school within 30 days from when your child first starts at the school, and when your child starts grades 9 and 11. If a copy is not given to the school within 30 days, the school will contact you. If your child has an appointment for an exam during this upcoming school year that is after the first 30 days of school (school will start 9/2/26, so after 10/2/26), please notify the Health Office of the date.

Vision and hearing screenings for students in grade 11 and scoliosis screening for boys in grade 9 are required. If not completed by a family practitioner, the screenings will be done by the school nurse under the direction of our school Medical Director.

A current updated physical (dated within a year of the first day of the sports season) will be needed to participate in sports in the fall of 2026.

Please check with your health care provider as soon as possible to make an appointment and work with us to meet this requirement. Health Examination forms can be submitted by email to Nurse Carey. For sports, please also include Mrs. Kaitlin Morris.

When to Keep a Child Home with Illness

It can be hard to know when to send children to school if they tell you that they do not feel well. Usually, the best place for them is in school, but there are some times when keeping them home to rest or calling for an appointment with your health care provider is recommended.

Please keep your child home and/or contact your child's doctor for:

• Fever greater than 100.4° (taken by mouth)
• Vomiting and/or diarrhea within the last 24 hours
• Severe sore throat along with fever and feeling ill for more than 48 hours, or after exposure to Strep throat infection
• Honey-crusted sores around the nose or mouth or rash on other body parts
• Large amounts of mucous (liquid) from their nose, with face pain or headache
• Severe ear pain or fluid coming from the ear
• Severe headache, especially with fever

If your child has a fever, it is not a good idea to give them medicine like Tylenol or Advil and send them to school because as soon as the medicine wears off, the fever may return and you will be called to come and pick up your child.

Please keep children home for 24 hours after the fever ends or they have completed 24 hours of medication if prescribed by your health care provider.

If you find your child is frequently asking to stay home from school, if they are falling behind or appear anxious about school, or if there does not appear to be any physical symptoms, contact your school nurse and your health care provider to discuss your concerns.

Remind children to throw away used tissues, cover their mouths when they cough or sneeze, keep their hands away from their face, and to wash hands often with soap and warm water. This will help keep everyone healthier.

FREE Student Athlete Cardiac Screening

St. Francis Hospital & Heart Center® (Catholic Health)

As part of our mission in the community, St. Francis Hospital conducts free cardiac screenings for student athletes. Students must be in grades 9 through 12. All students are required to have a consent form and medical history form completed by their parent prior to the evaluation. These forms can be found at CHSLI_org.

Proper picture ID is required and the student must also be accompanied by a parent. During the assessment, an EKG and echocardiogram will be performed. Results from the tests will be available immediately following the screening from the pediatric cardiologist.

Please call to schedule an appointment. Screenings are offered once a month during the school year.
