Dear Parent/Guardian:

As per federal regulations, we are required to send the following three directory information categories to military recruiters upon request: Student Name, Address and Telephone Number.

If you do not want to have this information released to military recruiters, you must inform us in writing by completing the information below.

Please return this sheet to the Guidance office by September 14, 2026 if you do not want this information released.
