Health and Dental Examination Requirements
Dear Parents/Guardians,
New York State law requires a health examination for all students entering the school district for the first time and when entering Pre-K, K, 1st, 3rd, 5th, 7th, 9th, and 11th grade. The examination must be completed by a New York State licensed physician, physician assistant, or nurse practitioner.
A dental certificate stating your child has been seen by a dentist or dental hygienist is also requested. A list of registered dental providers offering free or reduced-cost services is available upon request.
- Deadline: A copy of the health examination must be provided to the school within 30 days of starting school.
- Appointments: If the exam appointment is scheduled after the first 30 days of school, please notify the Health Office with the date.
- HIPAA/Consent: Healthcare providers cannot share health information with school staff without a signed consent form.

Mandated Health Screenings Letter
Dear Parent/Guardian,
The district's School Health Services program performs the following state-mandated health screenings during the school year:
- Vision Screening: Distance/near acuity for new entrants and Grades Pre-K, K, 1, 3, 5, 7, 9, and 11. Color perception screening for all new entrants.
- Hearing Screening: Required for new entrants and Grades K, 1, 3, 5, 7, 9, and 11.
- Scoliosis Screening: Required for girls in Grades 5 and 7, and boys in Grade 9.
A letter will be sent home with your child's exam results.
Medication Policy Guidelines
In accordance with NYS Law, the nursing staff must be made aware of all medications prescribed for your child.
- Prescription Medication: Requires original container labeled with student's name, physician's order, and written/signed parent request.
- Over-the-Counter Medication: Requires original container labeled with student's name, physician's order (as needed), and written/signed parent request.
- Inhalation Nebulizer Treatments: Requires labeled medication, tubing/nebulizer, physician's order with dosage/frequency, and written/signed parent request.
- Asthma Inhalers: Requires labeled medication and physician's order. Students with severe asthma may carry and self-administer inhalers only with specific written permission from physician, parent, and nurse.
Illness Guidelines: "When Is My Child Too Sick For School?"
Keep your child home if they have:
- Fever of 100°F or higher before taking fever-reducing medication.
- Vomiting or persistent diarrhea within the last 24 hours.
- Excessive tiredness or lack of appetite.
- Intense headache, sore throat, body aches, chills, or earache.
- Persistent, productive cough keeping them awake at night.
Return to School Criteria:
Your child may return to school once:
- Fever-free for 24 hours without fever-reducing medication.
- Vomiting/diarrhea-free for 24 hours without medication.
- On antibiotics for at least 24 hours (if prescribed).
- Normal appetite and energy levels have returned.
Pediculosis Capitus (Head Lice) Policy
Signs & Prevention:
- Signs: Persistent itching, nits (small silvery eggs attached to hair shafts near scalp/behind ears).
- Prevention: Inspect hair regularly, wash frequently, and discourage sharing combs, brushes, hats, or scarves.
Treatment & Readmission:
- Consult a physician or clinic for proper treatment and disinfect all contaminated household items.
- Notify the school nurse immediately (kept in strict confidence).
- Re-entry requires clearance through the Health Office with proof of treatment (doctor's note or empty box/bottle).
- Strict Requirement: REMOVAL OF ALL NITS IS REQUIRED BEFORE YOUR CHILD MAY RETURN TO CLASS.
Physical Education & Recess Excuse Procedure
Physical education is mandatory unless a medical condition warrants non-participation:
- Medical Note: A written note from your healthcare provider stating the condition and duration must be sent to the school nurse.
- Clearance: If the excuse duration is unspecified, written medical clearance is required before returning to physical education/recess.
- Parent Notes: A parent's written request is honored for a maximum of 3 days; beyond 3 days requires a doctor's note.
- Safety Policy: If a child cannot participate in Gym, they CANNOT participate in Recess.
