2026-2027 Community Eligibility Provision (CEP) Household Income Eligibility Form

West Babylon School District is participating in the Community Eligibility Provision (CEP). All children in a West Babylon District School can receive one free breakfast and one free lunch per school day at no charge regardless of household income or completion of this form. This form is to determine eligibility for additional State and federal program benefits that your child(ren) may qualify for. Read the instructions, complete only one form for your household, sign your name and return it to the School Lunch Dept by emailing it to Mrs. Dixon. Call the School Food Service Dept at 631-376-7750 if you need help.
