What does my child need to do to try out for a sports team?

  1. A current physical must be on record in the building health office. Physicals are valid for one year and expire on the last day of that month. For example, a physical dated Aug. 2, 2017 is valid through Aug. 31, 2018. In this case a high school student will be able to participate in a fall sport but must have an updated physical to participate in the winter or spring.
  2. A permission slip must be signed and returned to the health office within 30 days of the start of the season. You must do this for each season your child tries out for a sport.  
  3. A health history update must be on record with the health office within 30 days of the start of the season. You must do this for each season your child tries out for a sport. 

The school health offices continually schedule physicals throughout the year. Please check for dates on our website. If you miss these dates you must see your family doctor and drop off the completed forms to the building health office by the season due dates listed on the attached chart. All forms are available at the Athletic Office, Junior and Senior High School Health Offices and on line at the district web page www.wbschools.org under the Athletics tab.
